Refer someone to MySight York

If you are supporting someone living with sight loss and feel they would like additional practical, emotional, or community-based support, you’ve come to the right place.

Whether someone is newly diagnosed, finding everyday tasks more difficult, feeling isolated, or looking for specialist advice, MySight York offers a range of services designed to support people to live well with sight loss.

Is MySight York the right referral?

Before submitting a referral, please check you have answered yes to the following statements:


The person you are referring is living with sight loss or a visual impairment

The person you are referring lives within our service area (see coverage information below)

The person you are referring would like support with one or more aspects of living with sight loss

The person you are referring has given consent for this referral to be made

This referral might not be suitable if the person you are supporting requires a diagnosis or clinical care, lives outside of our catchment area, or has not given their consent. If you are unsure whether MySight York is the right organisation, please get in touch before completing the form. We will be happy to discuss the situation and advise on the most appropriate next steps.

Where we work

MySight York supports people living across York and the surrounding areas. Some services are only available within the City of York boundary, while others can also be accessed by people living in nearby areas.

Support is delivered in person, by telephone, online, depending on the service.

Eye Clinic Support - Sight support service

Best for: Someone who has recently received a diagnosis or is struggling to understand the practical and emotional impact of sight loss.
Coverage: York and surrounding areas

Counselling

Best for: Someone experiencing anxiety, low mood, loss of confidence, or emotional challenges linked to sight loss.
Coverage: York and surrounding areas

Equipment and Information Centre

Best for: Someone looking for practical solutions, equipment, information, or advice to make everyday life with sight loss easier.

Digital Inclusion

Best for: Someone who would like support using technology, apps, or other assistive devices.

Activities and Groups

Best for: Someone who would like to stay connected, meet others, learn new skills, or take part in accessible activities.

Befriending and Home Visiting

Best for: Someone experiencing loneliness, isolation, or a loss of confidence.

Making a referral


The form will ask you to provide some basic information about the person you are referring. Please complete it as accurately as possible.

This helps us ensure the person receives the most appropriate support and allows us to contact them in the way that works best for them.

What happens after you make a referral?


Step 1: Referral received

We will confirm receipt of your referral and review the information provided, making sure we have everything we need before accepting it.

Typical timeframe: Within 5 working days

Step 2: Allocation

The referral is passed to the most appropriate member of our team, who will assess the person’s needs and identify the right support options.

Typical timeframe: Within 2 working weeks

Step 3: First conversation

We will contact the person referred, using their preferred method of communication, to chat through the available support options and next steps.

Typical time frame: directly after the review stage is complete
